I've seen these issues manifest in Gnome software and a small percentage of people contribute to the stack for screen readers and related tech. So Gnome cannot get to the minimal level we expect.
GPL is open but with "restrictions" to keep it open. We need Communities and "tech" to foster open communities but with "restrictions" to keep it open.
Creating open communities is a lot harder than creating open IP.
