Software managers and their bosses never like to hear that the best answer is to start over, so they'll ignore such advice in favor of slipped schedules, undelivered promises, and unmaintainable software; then in ten years, they'll start over, often after the people wise enough to suggest it are long gone.

Of course, it's hard to blame them. The software development world continues to treat software as magic, not science, so there's no empirical evidence pointing toward the best development methods.
